(This is set up to access a site called HEAVEN -- you should alter the HTML to reference a site whose information you have created using the form interface)

NB: Regardless of this example, you'd be NUTS to give ordinary users the ability to specify anything other than a GatherList and (maybe) a password. In particular, OriginPath and Sites should be safely locked away from their tampering!
